To apply, order forms D2 and D4 from the Driver and Vehicle Licensing Agency (DVLA). The D4 form has to be filled in by a doctor. This could be either: (i) your GP - but an optician might need to fill in thesection about your eyesight (ii) a private firm specialising in drivers' medical exams 2: Be over 18 18+ YEARS Order the forms online If you are living in UK then download form from below given link; https://forms.dft.gov.uk/order-dvla-forms/ 3: Get a professional driving qualification called the Driver Certificate of Professional 2: Pass the 4 tests that make up the Driver CPC to qualify Competence (CPC) The Driver CPC initial qualification has four parts: You'll get your Driver Certificate of Professional Competence (CPC) card when you've done 35 hours of periodic training. (i) theory test (this includes two separate tests - multiple choice and hazard perception) (ii) case study test (iii) driving ability test (iv) practical demonstration test 12 Honth bafaue your doadlina You must pass all four parts to get your Driver CPC. When you'll be sent your card You'll get your new Driver CPC card straight away if you complete your training in the 12 months before your deadline, If you complete your training more than a year early You will not get your new Driver CPC card until nearer the time your current card expires 3: Take 35 hours of periodic training every 5 years to stay qualified.
