Step 1a: If you are a family member, notify the physician treating the patient at the current hospital of the transfer request, and gather the physician's contact information. Step 1b: If you are a referring physician, please proceed directly to Step 2. Step 2: Call Mount Sinai Transfer Service @ 1-800-TO-SINAI and provide the following: Referring physician and/or hospital name • Phone number where you can be reached • Patient location and phone number • Services needed Accepting physician's name (if known) Patient's name Diagnosis •Special needs (isolation, ventilator, or IVs) Step 3: A Mount Sinai Transfer Service coordinator will connect a Mount Sinai physician with the current physician to discuss and accept the transfer. H. Sending Hospital Mount Sinai Step 4: After the two physicians agree on the transfer, the coordinator will gather demographics and insurance information from the sending hospital. Step 5: A Mount Sinai Transfer Service coordinator will coordinate the insurance approval, bed placement, and ambulance transportation. Step 6: The patient is transferred from the sending hospital to The Mount Sinai Hospital.
